Overview

ADRs4AI is a method and set of tools for saving the reasoning behind decisions you make with an AI model. The model writes decision records and open questions with options, a recommendation, the rationale, its confidence and a test that could show it is wrong, and a person accepts or revises them in the repository. It comes as a repository template, a Claude Code plugin, a VS Code extension with an MCP server that agents can query, and an iOS app for answering questions.
