Write teach-back questions to check what a patient understood
A prompt for nurses running a teach-back check after teaching a patient about a condition, medicine or procedure. It writes five open questions where the patient explains things in their own words without feeling quizzed, notes what a full answer covers and what a partial one means you should reteach, and adds a brief, two-sentence restatement of the point most often misunderstood. Wording is kept to roughly a sixth-grade reading level. A nurse should check the questions before using them with a patient.
